How to Obtain Test Accomodation

A student is required to be registered with the Disability Resource Center (DRC) prior to receiving services. Recent information that documents the presence of a disabling condition and verifies the student's request for services is required from a qualified professional. Check with the DRC Access Consultant for information.

Steps

  1. Meet with the DRC Access Consultant for approval and recommendations on accommodations.
  2. Complete a separate Testing Authorization form (Gold Sheet) for each class.
  3. Take the Testing Authorization Form to your instructor and have them double check the dates, identify time allowed in class for exam, and sign the back of the form.
  4. Return the signed form to the Services Coordinator.
  5. Maintain a copy for yourself so you can enter the dates into your calendar.
  6. It is your responsibility to confirm the date and time of all exams scheduled with the Services Coordinator at the DRC.
  7. You are responsible for notifying the Services Coordinator of any changes in your exam schedule that are made by the instructor. Any changes made by you will require your instructor's permission to reschedule the exam. You must fill out a Change in Testing Authorization (Green Sheet) and submit it to the Services Coordinator three working days before the exam; ten working days before the end of the semester for a final.
  8. Make sure that you arrive at the DRC office a few minutes before your scheduled exam time.

Do's and Don'ts

Do's

  • Do contact the Services Coordinator a minimum of five (5) working days in advance if any changes occur to the original schedule you submitted.
  • Do arrive on time. If you are late, we will not extend the time because of your lateness. Time lost is yours!
  • Do fill out each form completely.

Don'ts

  • Don't attempt to bring any unauthorized materials into the testing area.
  • Don't bring food into the testing area, unless you are testing over lunch break or have health reasons requiring you to eat at certain times.

Examination Information

During the examination, you will be monitored by a Proctor. If you are found to have any unauthorized materials in your possession, the examination will be stopped immediately, and you will be required to meet with the DRC Director. The professor will be notified of the situation, and he or she will decide the appropriate punishment. The result of cheating on a test can be as severe as dismissal from the university. If a student is found to be cheating on an examination while using test accommodations in the DRC, the need for accommodations will be reevaluated by the DRC Director.

If you have a question regarding the examination, ask the Proctor to contact the DRC Services Coordinator. An attempt will be made to contact your Professor or an individual who can respond to your question.

If you need to use the restroom during the examination, notify the Proctor who will accompany you to the restroom. You will not be permitted to bring any materials with you to the restroom.
